You use your lungs when you breathe. The air you take in through your nose or mouth flows down the trachea (windpipe). The trachea divides into two tubes called the left and right bronchi, which carry air to each lung. Once inside the lung, the bronchi divide into smaller and smaller tubes called bronchioles. Each bronchiole ends in a cluster of tiny air sacs called alveoli. The alveoli take oxygen from the air you breathe in and pass it into the blood for circulation to all parts of your body. The alveoli also remove carbon dioxide from the blood, which is pushed out of the lungs when you exhale. So now you understand the main purpose of your lungs... and now to a closer look at lung cancer.

What most people don't know is that there are two types of lung cancer. The most common type is the non-small cell , also known as (NSCLC). This cancer grows more slowly than the small cell type. While the small-cell grows quickly and often spreads to distant parts of the body.

There is also a rare type of cancer called pleural mesothelioma which is often mistakenly called a lung cancer. But pleural mesothelioma starts in the lining of the lung and is very different from cancer that starts in the lung.

Now for the three types of non-small cancers, which are squamous cell carcinoma, adenocarcinoma, and large cell carcinoma. These account for about 80% of lung cancers, while small-cell (also called oat) accounts for about 20%. The vast majority of all lung cancers are of the carcinoma type.

It's important to understand these three types of cancers because treatment varies accordingly. Non-small cell lung carcinoma for example, is sometimes treated with surgery, while small cell lung carcinoma, usually responds better to chemotherapy and radiation.

Photodynamic therapy (PDT) is one of the newer lung cancer treatment options available today. Lung cancers are sometimes inoperable, particularly when they are situated in the bronchi or trachea. Similarly, cancers that have spread from other parts of the body to the bronchi cannot be removed surgically. In such cases, photodynamic therapy offers a safe and effective treatment option.

In a best-case scenario, PDT is used as a curative therapy that can eliminate the cancer completely. In many patients, PDT is employed as a palliative lung cancer treatment meant to provide relief from symptoms even though it does not cure the cancer.

Photodynamic therapy can effectively relieve symptoms like breathing difficulties from obstructions in the bronchi or trachea. In some patients, tumors block the airways and can cause coughing, breathing difficulties, pneumonia and bleeding. PDT may be used in such cases depending on the location, size and stage of the tumor.

PDT destroys cancer cells using a laser. To ensure that only cancerous cells are destroyed, a special drug is used to make the cancer cells more sensitive to this treatment while leaving normal cells intact.

There are three steps involved in the process of lung cancer treatment using PDT. In stage one, a drug called photofrin is injected intravenously. Also, the tumor must be large enough so that the bronchoscope can easily detect it. This means that some cancers like those located outside the bronchial passages cannot be treated with PDT.

Lung cancer treatment with PDT offers almost no dangerous side effects, unlike other treatment methods. Photosensitivity is about the only side effect that lasts for a while. During this time, even normal exposure to sunlight can result in severe sunburn. * Context.-Signal transducer and activator of transcription 3 (STAT3) is constitutively activated in diverse human cancers and plays a critical role in tumor cell survival, proliferation, migration, invasion, angiogenesis, and inhibition of apoptosis. The phosphorylated active form of STAT3 (pSTAT3) mediates its effects via nuclear transcriptional activity. However, it was recently observed that the nonphosphorylated, cytoplasmic, inactive form of STAT3 is involved in cell motility and consequently tumor invasion. It appears that, although STAT3 is not absolutely required for tumor formation, tumors that develop in the presence of STAT3 become dependent on its expression for their survival, making it a potential therapeutic target.

Objective.-To investigate the possible utility of STAT3 as a future therapeutic target in non-small cell lung carcinoma (NSCLC) and malignant mesothelioma (MM).

Design.-Immunohistochemical expression of MIB-1, STAT3, and pSTAT3 was assessed in 303 NSCLC and 44 MM archival cases.

Results.-A more conspicuous expression of inactive STAT3 (91.44% in NSCLC and 79.5% in MM cases) was present compared with the nuclear activated form pSTAT3 (60.53% in NSCLC and 61.4% in MM cases). MIB-1 did not correlate with the expression of STAT3 or pSTAT3.

Conclusions.-The strong expression of cytoplasmic inactive STAT3 in NSCLC and MM cases implies a major role for STAT3 in tumor motility, invasion, and metastasis via a nontranscriptional pathway. We conclude that STAT3 and pSTAT3 are up-regulated in a high percentage of NSCLCs and MMs, regardless of tumor type, age, sex, smoking status, stage and grade of tumor, or survival, providing a basis for therapeutic intervention.

Signal transducer and activator of transcription (STAT) proteins are latent cytoplasmic molecules (Figure 1, A), which are activated in response to many cytokines (eg, interleukin 6) and growth factor signals1 (eg, epidermal growth factor, platelet-derived growth factor, and vascular endothelial growth factor) (Figure 1, B) and have the ability to transduce a signal from a cell surface receptor into the nucleus and promote the transcription of specific genes. Although these transcription factors are often not mutated themselves, their overactivation by mutations in other genes leads to the gene expression changes that are hallmarks of malignant disease. The activation of cytoplasmic, latent STATs is dependent on tyrosine phosphorylation (Figure 1, C), which induces dimerization (Figure 1, D) via interaction between 2 STAT molecules.2 Many tyrosine kinases including Janus kinases (Figure 1, E) and receptor tyrosine kinases (Figure 1, F) can mediate phosphorylation (activation) of STAT proteins. Activated STATs are translocated to the nucleus (Figure 1, G), where they bind to consensus sequences on the promoter of target genes and activate their transcription2 (Figure 1, H). In normal cells, STAT tyrosine phosphorylation is transient, lasting from minutes to several hours, and the tyrosine dephosphorylated STAT proteins are shuttled back to the cytosol from the nucleus3 (Figure 1, I). However, in numerous cancer-derived cell lines or in primary tumors, STAT proteins are persistently tyrosine phosphorylated.4 There are at least 6 STAT proteins involved in this complex signaling pathway. Among these STATs, STAT3 stands out by its constitutive activation in most human neoplasms.2,5 Activation of STAT3 is detected in solid tumors (breast, lung, head and neck [squamous cell carcinoma]; ovarian and pancreatic cancers; melanoma; hepatocellular carcinoma; cholangiocarcinoma; and prostate, endometrial, and cervical cancers) as well as in hematopoietic tumors (acute myelogenous leukemia, multiple myeloma, Hodgkin disease, non-Hodgkin lymphoma, B-cell lymphoma, and cutaneous T-cell lymphoma).1,2,5-7 It has been demonstrated that STAT3 in tumor cells promotes cell growth and cell survival through the transcriptional up-regulation of target genes7 (eg, c-myc, bcl-xL, bcl-2, cyclin D1, VEGF), whose products promote cell proliferation, cell migration, invasion, angiogenesis, and inhibition of apoptosis8 (Figure 1, J). It has been described that the inhibition of STAT3 function in non-small cell lung carcinoma (NSCLC) cells increases apoptosis of these cells, suggesting that STAT3 is necessary for their survival.9,10 It is believed that STAT3 mediates its effects through the nuclear localized transcriptional activity. However, evidence for a cytoplasmic nontranscriptional role for STAT3 in cell motility and consequently in tumor invasion has accumulated and a direct effect of nonphosphorylated cytoplasmic STAT3 on cell motility through a nontranscriptional protein interaction mechanism has been recently demonstrated11,12 (Figure 2, A through E). STAT3's control of cell motility, in addition to its cell proliferation-promoting and antiapoptotic activities, directly contributes to its role in tumorigenesis and tumor progression.8 In addition, STAT3 has been described as a component of focal adhesions (sites of cell contact with the extracellular matrix) that may contribute to the invasiveness of cancer cells13 (Figure 2, C). There is an unmet need to develop new treatment modalities for lung cancer. Molecular targets for treating cancer have been particularly difficult to develop because of the multiplicity of genetic mutations in cancer cells. Targeting 1 mutation at a time appears to be ineffective. Recently, however, it has been realized that genetic mutations in cancer often converge on central regulators: the transcription factors.5 New research is now pointing to STAT3 as a promising target for the development of a global anticancer therapy agent. To investigate the possible utility of STAT3 as a future therapeutic target in NSCLC and malignant mesothelioma (MM), we evaluated the presence of the inactive form of STAT3, the active phosphorylated form of STAT3 (pSTAT3), and the proliferation marker MIB-1. In NSCLC cases, the results were correlated with age, sex, grade, stage, smoking status, and survival.

Lung cancer may also be the most tragic cancer because in most cases, it might have been prevented, 87% of lung cancer cases are caused by smoking. Lung cancer is the second most commonly occurring form of cancer in most western countries and although the lung cancer incidence is less common in developing countries, the rapid increase in the popularity of smoking will see the number of lung cancer sufferers in those countries quickly catch up with the western world.

Lung cancers can arise in any part of the lung, and 90%-95% of cancers of the lung are thought to arise from the epithelial, or lining cells of the larger and smaller airways (bronchi and bronchioles); for this reason, lung cancers are sometimes called bronchogenic carcinomas or bronchogenic cancers.

The most common type of lung cancers are epidermoid carcinoma, small cell carcinoma, adenocarcinoma and large cell carcinoma.

Most experts agree that lung cancer is attributable to inhalation of carcinogenic pollutants by a susceptible host. Who is most susceptible? Any smoker over the age of 40, especially if they began smoking before the age of 15, have smoked 20 or more for 20 years, or worked with or near asbestos. Two other factors also increase susceptibility: exposure to carcinogenic industrial and air pollutants (asbestos, uranium, arsenic, nickel, iron oxides, chromium, radio active dust, and coal dust.) and familial susceptibility.

SIGNS AND SYMPTOMS

Because early lung cancer usually produces no symptoms, the disease is often in an advanced stage when first diagnosed. Late stage signs are: with epidermoid and small cell carcinoma; smokers cough, hoarseness, wheezing, dyspnea, hemoptysis and chest pain. METASTASES

Lung cancer that spreads to the brain can cause difficulties with vision and weakness on one side of the body.

Lung cancer may grow into certain nerves in the neck, causing a droopy eyelid, small pupil, sunken eye, and reduced perspiration on one side of the face; together these symptoms are called Horner's syndrome (see Autonomic Nervous System Disorders: Horner's Syndrome). Lung cancer may grow directly into the esophagus, or it may grow near it and put pressure on it, leading to difficulty in swallowing. TREATMENT

Treatment for lung cancer depends on the cancer's specific cell type, how far it has spread, and the patient's performance status. If investigations confirm lung cancer, CT scan and often positron emission tomography (PET) are used to determine whether the disease is localised and amenable to surgery or whether it has spread to the point where it cannot be cured surgically. Treatment is usually a combination of surgery, chemotherapy and radiation therapy.

Surgery is usually the first option. Chemotherapy can be used as a first line treatment for lung cancer or as additional treatment after surgery. It was used in some building materials until 1999; however most was phased out in the early 1980s. The British Lung Foundation survey relating to the fiber concentrated on builders, carpenters, plumbers, electricians, joiners and gas fitters. The survey found one in 10 had felt under pressure to continue working even if they had not seen a survey for the building and suspected that the dangerous fiber was present. In addition, nearly a third of those in the survey mistakenly believed that they did not need to see any types of asbestos survey because it had already been removed from all British buildings.

The majority of tradesmen - 81% in the British Lung Foundation's asbestos survey - said they never, rarely or only sometimes asked if the site they were working on had been checked for the fiber via an official asbestos survey, before they started a job - even though it is a legal requirement for them to see a survey which informs them of the presence of the dangerous mineral fiber.

Under the Control of Asbestos Regulations 2006, it is a legal requirement that all those that may disturb the fabric of any building must have Asbestos Awareness Training. This form of awareness training for asbestos helps tradespeople increase their general awareness of the fiber within their working environment.
It has been estimated that as many as 20 tradespeople die every week as a result of exposure to this fiber in their workplace. In fact exposure to asbestos has been deemed as one of Britain's biggest workplace killers.

So why isn't awareness training a number one priority for all tradespeople because with better training they would reduce their risks of asbestos related lung disease and mesothelioma. However if the British tradespeople continue their ignorance to the dangers of asbestos and don't put their own health first then campaigns, like those run by the British Lung Foundation, to eliminate asbestos related cancers such as mesothelioma and make them a disease of the past, are a lost cause.

So lets all strive to make our British tradespeople more aware of the dangers of asbestos by making sure asbestos awareness training is a core skill they all own and all tradespeople are presented with an official survey before they start a job on a particular site.

Grandparents on both sides of my family died from lung cancer and an Aunt too. My grandparents and aunt smoked for years and lung cancer was the result. Many people get cancer and never did anything, at least that they were aware of, to get it.

However they may have gotten it, and whatever type of cancer it is, it affects those who love them. But if you are a smoker, stop it. I know it's hard, I smoked too, but if you don't stop, you could get lung cancer yourself and die. Is that worth your next puff? Think about who will miss you. My grandfather is the one I saw up close. He also had throat cancer and had his larynx removed. He went from being a strong active man to a withered shell who couldn't talk. It was very sad.

Some people go through life and don't have someone they love get cancer, but it's rare. I have many friends who have gone through breast cancer, colon cancer, prostate cancer, and Non-Hodgkins lymphoma, and they have all survived. I celebrate their time still here.

Certainly time has made a difference as cancer treatment was not as advanced when my grandparents had cancer.

Those who get cancer are not the only victims. Cancer impacts families and friends too. If you are a smoker, seriously, consider quitting. If you know a smoker, don't nag them to quit, just tell them you love them.

Cancer is bad, do what you can to avoid it, and if someone you love gets it, cherish them and the time you have with them. That's good advice even without cancer. Cherish those you love, and tell them.

"If CT scans and other tests indicate that it's too late for surgery (because the cancer has already spread to the glands in the opposite side of the chest or elsewhere), I leave it alone if it's not causing any symptoms. I prescribe radiation, surgery, or chemotherapy only if the incurable cancer is causing symptoms. The purpose of intervention at this point is to make the person feel better. When a patient with advanced lung cancer has no complaints, I have them do nothing. This approach is not shared by all or even most doctors. The majority of my colleagues go full-blast with radiation, surgery, and toxic chemotherapy in the hope of obtaining a cure, which rarely, if ever, happens at that stage. Gudkov and a team of scientists use the drug, CBLB502, to block apoptosis in the same manner as cancer, however, they block the suicide of healthy cells. This causes healthy cells to regenerate rather than deadly cancerous cells that develop from radiation and other forms of exposure to cancer-causing materials.
